What is Ksharasutra?
Ksharasutra is a specially prepared medicated linen thread, coated layer-by-layer with potent herbal preparations. It works through a combination of chemical cauterization and mechanical action — gently cutting, draining and healing diseased tissue while preserving the surrounding healthy anatomy.
This technique is now widely practiced not only in India but also in various other nations as a preferred alternative to conventional fistula and pile surgery.
How It Is Made
Ksharasutra Preparation — Step by Step
A meticulous 21-coating process across three stages to create the medicated therapeutic thread.
First Stage
Snuhi Latex Coating
- Take a sterile surgical linen thread.
- Apply Snuhi latex uniformly on the thread.
- Hang the thread in a drying cabinet.
- Repeat the coating and drying process 11 times.
Second Stage
Kshara Coating
- Apply Snuhi latex again on the thread.
- Roll the thread in Apamarga Kshara powder.
- Dry the thread in the cabinet.
- Repeat this process 7 times.
Third Stage
Haridra Coating
- Apply Snuhi latex on the thread.
- Roll the thread in Haridra (turmeric) powder.
- Dry the thread properly.
- Repeat this process 3 times.

Application
In Fistula-in-ano
Following the administration of local, spinal, or general anesthesia, the patient undergoes a procedure wherein a flexible probe is inserted through the external opening of the fistula, traversing to the internal opening in the anal canal. Subsequently, the probe is gently guided out through the anal opening, carrying with it a Ksharasutra positioned within the probe's groove. The two ends of the Ksharasutra are then securely tied together. After a week, the Ksharasutra is replaced with a new one. Over time, the Ksharasutra gradually cuts and promotes healing of the fistulous tract. Ultimately, the entire fistulous tract heals following the cutting process.
